Relative to adrenal insufficiency after cardiac arrest spontaneous circulation recovery, poor Kim Kim et al reported that for patients with spontaneous circulation recovery after cardiac arrest and resuscitation, relative adrenal insufficiency may be associated with increased mortality. [Am J Emerg Med 2006, 24(6): 684].
The prospective cohort study included 30 patients, 15 males, and an average of 52 years. The cause of cardiac arrest in most patients is unknown, and the most common initial heart rhythm is cardiac arrest. The results showed that the average time from cardiac arrest to cardiopulmonary resuscitation was 8 minutes, and the average time to spontaneous circulation recovery was 23 minutes. The incidence of relative adrenal insufficiency was 43% and 33% in the initial 12-24 hours and 24-48 hours, respectively. The level of basic hydrocortisone was normal or elevated in all patients. Nineteen (58%) patients survived, and only 5 (26%) had relative adrenal insufficiency.
The study suggests that although the level of basal hydrocortisone in spontaneous circulation recovery after cardiac arrest is not low, relative adrenal insufficiency may occur, and the use of glucocorticoids may reduce patient mortality. ——Midi Medical Network
