Insufficient diagnosis of male prostatitis and interstitial cystitis

Dr. Anne Suskind of the University of Michigan reported at the American Urological Association (AUA) annual meeting that interstitial cystitis and chronic pelvic pain in men may be more common than expected. The national telephone survey shows that about 2 million American men suffer from one of these diseases.

Suskind and colleagues conducted a survey with the RAND IC Epidemiological Study (RICE) in women (the largest epidemiological study of interstitial cystitis to date and found that up to 6.5% of American women may have interstitial cystitis) Similar telephone surveys. This male-targeted survey used the validated RICE version definition to assess men's issues. Chronic prostatitis is defined as the National Institutes of Health's chronic prostatitis index >5, with ejaculation or perineal area pain. The researchers contacted 6,072 families and asked whether male residents were willing to accept the survey on President Obama’s performance. The results showed that initially there were 296 males with positive bladder symptoms, and 149 of them met the diagnostic criteria for interstitial cystitis or prostatitis. Of these 149 patients, 52 were excluded from the final analysis. Of the remaining 97 subjects, 23% met the definition of high specificity for interstitial cystitis/bladder pain syndrome, 16% met the case definition of chronic prostatitis/chronic pelvic pain syndrome, and 8% met both criteria. Based on the above results, Dr. Suskind estimates that up to 2 million people in adult males in the United States meet the diagnostic definition of any of these diseases. Further analysis showed that the overlap between the two conditions was very low, only 17%, suggesting that the two diseases are easily distinguishable at the time of diagnosis.
