Shanghai "breaking problem" pediatrics difficult to see a doctor: develop APP to high-risk asthma

For two consecutive years, the problem of “difficulty in seeing a doctor” in pediatrics has become the topic of “two sessions” that Shanghai Municipal Party Committee Secretary Han Zheng is most concerned about. At the two sessions in Shanghai in 2016 and 2017, Han Zheng gave the hospital directors an "arch" of the difficult problem of pediatric medical treatment. At the Shanghai two sessions that just closed this year, Han Zheng once again mentioned that “to pay more attention to pediatricians, so that every child can get convenient, high quality, balanced and continuous medical services.”

The reporter noted that this ideal and in place pediatric medical service has already had some eyebrows in some grassroots communities in Pudong New Area, Shanghai. The number of emergency departments in the Pediatric Leading Hospital and Shanghai Children's Medical Center has decreased, while the growth rate of pediatric outpatient clinics in community health centers and secondary hospitals has been “overtaken”.

Doctors develop apps to "highly open" asthma

In the community health center of Heqing Town, Pudong New Area, at least 10 parents of children have installed the app for asthma specialists developed by the respiratory specialist and technical team of Shanghai Children's Medical Center (hereinafter referred to as “Child Medical Center”). Asthma has no worries." This software can record the child's medical treatment process every time the child goes to the community hospital for airway resistance, lung function and other tests - the number of asthma attacks per month, the number of emergency operations, the frequency of antibiotic use, the length of treatment, Changes in lung function status, etc.

The client is divided into a medical version and a patient end. On the one hand, it helps the patient to scientifically manage the disease. On the other hand, it also facilitates the referral of the child with a serious condition to the child care center.

Yin Yong, director of the Department of Respiratory Medicine at the Pediatric Center, told reporters that at present, asthma has become a high-risk disease among children in Shanghai, and the incidence rate is 10% high. Since its opening in May last year, the “Asthma Worry” platform has covered towns and villages such as Mucheng Town, Da Tuan Town, Huinan Town, Zhuqiao Town, Heqing Town, Hudong New Village Street and Yangshuo Street in Pudong New Area. More than 510 children registered in the platform and population in the third quarter of Pudong.

In the past three months, Yin Yong has classified the two types of patients registered on the App—one group of patients who have asthma management in the community and the other who are treated with asthma in a paediatric center. In the three months, the asthma control rate was the same in both groups, and the community hospitals spent less and spent less time on the patients. “It’s more convincing than sending one or two experts to the community, spending less. The money and time to enjoy the same quality of service is very good and affordable for the people."

Adjacent to Pudong Airport's Zhuqiao Community Health Service Center, the service covers 71 village committees and has about 18,000 children aged 0-14. Children with asthma go to the pediatric center at least once a month for a long treatment period, which takes 4 hours for each run.

Today, asthma experts at the Pediatric Center are stationed in the community, not only have standardized construction programs, but also unify the standard of referrals. High-risk children were screened in the community childcare clinic through an international scale, and referred to the outpatient asthma specialist through the green channel. After being diagnosed by a specialist and developing a control plan, they are transferred back to the community for management and follow-up.

This community health service center now has 276 children with asthma, including more than 50 regular follow-ups. Yin Yong introduced that the 2017 “Asthma Worry” program will absorb 700 children's data.
